Case Study 16

-->
Jonathan drives a 4-axle bulk tipper 5 days per week. He carries a variety of animal foodstuffs to farms within a 60 mile radius of the feed mill, which is his base. His vehicle is fitted with an 'easy sheet" to facilitate safe loading and a reversing camera.
On Saturday he volunteers as a coach for a local under 13 football team and generally spends about 5 hours with the team.

There are 10 multiple choice questions in this case study. Read this carefully and ensure you fully understand the scenario before starting the test. You need to score 8 out of 10 to pass.

A Damage your mental health, causing depression etc.
B Help reduce stress and fatigue and make you feel better about yourself
C Make you put on weight because of the extra muscle gain
D Make you tired, especially if you have a sedentary job like driving
Correct Answer: B Help reduce stress and fatigue and make you feel better about yourself
Explanation: • Damage your mental health, causing depression etc.: No. quite the opposite, exercise has been shown to be beneficial to mental health.
• Make you tired, especially if you have a sedentary job like driving: No. on the contrary it generally feels invigorating.
• Make you put on weight because of the extra muscle gain: No.
• Help reduce stress and fatigue and make you feel better about yourself: Yes.
A Because he must leave his vehicle when taking a break after 4.5 hours of driving
B To check for any debris which cannot be seen via the reversing camera
C To check for any soft ground that may cause the vehicle to overturn as the weight is transferred rearward during tipping
D To check for any soft ground that the vehicle may sink into
Correct Answer: A Because he must leave his vehicle when taking a break after 4.5 hours of driving
Explanation: The following are all incorrect answers, because the question asks for the one item that is NOT a good reason for leaving the cab before reversing:
• To check for any soft ground that may cause the vehicle to overturn as the weight is transferred rearward during tipping: The driver will look to see that the ground is firm and able to take the weight of the vehicle and load.
• To check for any soft ground that the vehicle may sink into: The driver will want to avoid getting the rear (drive) axles stuck in any soft ground.
• To check for any debris which cannot be seen via the reversing camera: Old metal or rocks can damage wheels and tyres and may not be obvious in the mirrors or reversing camera.
This answer is correct because it is NOT a valid reason for Jonathan to leave the cab:
• Because he must leave his vehicle when taking a break after 4.5 hours of driving: The driver could take a break in his vehicle but, in any case, we are not told that the driver has driven for 4.5 hours - and all the other options are valid reasons to leave the vehicle prior to reversing.
Correct Answer: A Bread, cereals and pasta
Explanation: • Chocolate: No.
• Crisps: No.
• Meat and fish: No, these are part of a balanced diet, but they are not foodstuffs that provide sustained energy over a period of time.
• Bread, cereals and pasta: Yes, whole grain varieties of bread and cereal are best
A A small amount of food and drinks containing high fat or sugar levels
B Plenty of dairy products
C Plenty of fruit and vegetables
D Plenty of starchy foods bread etc.
E Some meat, fish, eggs and beans
Correct Answer: B Plenty of dairy products
Explanation: The following are all recommended items on the eat well plate, so are not the correct answer to this question:
• Plenty of fruit and vegetables.
• Plenty of starchy foods bread.
• Some meat, fish, eggs and beans.
•A small amount of food and drinks containing high fat or sugar levels.
This item is not a recommendation on the eat well plate, so is the correct answer to this question:
• Plenty of dairy products: No. it suggests only a small amount of these.
A He does not need to record this activity
B Make a manual entry in his diary
C Make a manual entry on an analogue tachograph chart
D Make a manual entry on the instrument on return to work on Monday
Correct Answer: A He does not need to record this activity
Explanation: • Make a manual entry in his diary: No, he does not need to record this activity as it is being carried out when he is "free to dispose of his time' and it is therefore regarded as rest.
• Make a manual entry on an analogue tachograph chart: No, he does not need to record this activity as it is being carried out when he is "free to dispose of his time' and it is therefore regarded as rest.
• Make a manual entry on the instrument on return to work on Monday: No, he does not need to record this activity as it is being carried out when he is "free to dispose of his time" and it is therefore regarded as rest.
• He does not need to record this activity - Correct, he does not need to record this activity as it is being carried out when he is "free to dispose of his time" and it is therefore regarded as rest.
A Inhaling dust from the product, as it is unloaded
B Other vehicles reversing in the vicinity
C Overhead cables and the unpropped body
D Soft ground, causing the vehicle to roll or tip
Correct Answer: C Overhead cables and the unpropped body
Explanation: • Other vehicles reversing in the vicinity: No - this is something that Jonathan needs to be aware of, but is not the main risk here.
• Inhaling dust from the product, as it is unloaded: No, from his position at the controls, Jonathan is well away from the source of the dust, but he may need to wear appropriate PPE in any case.
• Soft ground, causing the vehicle to roll or tip: No, the vehicle is parked on hard ground.
• Overhead cables and the unpropped body: Yes, the major hazard here is the overhead cable as, once he starts to raise the body, it will be difficult to see how close it is to the raised part of the truck. Overhead electricity cables could "arc" across to the truck well before the body touches them. The unpropped body is a potential hazard since Jonathan is outside the vehicle and close to the vehicle.
A Check for soft ground
B Check the vehicle lights are on and that the rear doors are locked
C Ensure that the handbrake is fully applied
D Release the tension in the sheet
Correct Answer: B Check the vehicle lights are on and that the rear doors are locked
Explanation: These answers are all incorrect because these are checks that DO need to be made before raising the body and the question asks for checks that do NOT need to be made:
• Check for soft ground: If the vehicle is standing on soft ground it will become increasingly unstable when the body is lifted. It is good practice to check this.
• Release the tension in the sheet: This will more readily allow air to enter the load cargo area as the load is emptied and reduce the risk of a vacuum being formed, so it is good practice to release the tension in the sheet.
• Ensure that the handbrake is fully applied: This will prevent the vehicle moving forward as the load is delivered.
This answer is correct, because it contains checks that do NOT need to be made:
• Check the vehicle lights are on and that the rear doors are locked: The doors will need to be unlocked to allow the product to escape and whether the lights are on or off is not relevant to the raising of the body.
A Cause the centre of gravity to become higher and thereby decrease the risk of the vehicle turning over
B Cause the centre of gravity to become higher and thereby increase the risk of the vehicle turning over
C Cause the centre of gravity to become lower and thereby decrease the risk of the vehicle turning over
D Cause the centre of gravity to become lower and thereby increase the risk of the vehicle turning over
E Not affect the centre of gravity of the vehicle
Correct Answer: B Cause the centre of gravity to become higher and thereby increase the risk of the vehicle turning over
Explanation: • Not affect the centre of gravity of the vehicle: No. this is incorrect, this action will have a marked effect on the centre of gravity of the vehicle.
• Cause the centre of gravity to become higher and thereby decrease the risk of the vehicle turning over: No. it will cause the centre of gravity to become higher thereby INCREASING the risk of the vehicle overturning.
• Cause the centre of gravity to become tower and thereby decrease the risk of the vehicle turning over: No. it will cause the centre of gravity to become higher thereby INCREASING the risk of the vehicle overturning.
• Cause the centre of gravity to become tower and thereby increase the risk of the vehicle turning over: No. the centre of gravity will rise with the toad thereby reducing the stability of the vehicle and making it more susceptible to overturning.
• Cause the centre of gravity to become higher and thereby increase the risk of the vehicle turning over: Yes. as the centre of gravity becomes higher (because the toad is now further from the ground) it will increase the risk of the vehicle overturning.
A Carry on tipping the body in the hope that it will settle as the load is released
B Jump from the cab
C Stop the tipping action and try to drive the vehicle forward slowly to firmer ground
D Stop the tipping action and try to slowly lower the body
Correct Answer: D Stop the tipping action and try to slowly lower the body
Explanation: • Jump from the cab: No, this would not be safe and could lead to injury or death by crushing.
• Carry on tipping the body in the hope that it will settle as the load is released: No. it will cause the centre of gravity to become higher thereby INCREASING the risk of the vehicle overturning.
• Stop the tipping action and try to drive the vehicle forward slowly to firmer ground: No. any movement of the vehicle is likely to increase the danger.
• Stop the tipping action and try to slowly lower the body: Yes.
A Call a fitter and contain the escaping fluid by means of a spill kit or other facility
B Get some water, and wash the spilt hydraulic fluid away
C Lower the body and call a fitter
D Try to stem the leak using tape or other waterproof material
Correct Answer: A Call a fitter and contain the escaping fluid by means of a spill kit or other facility
Explanation: • Lower the body and call a fitter: No, this would not be possible as the body would either be locked into position by the non-return valves or more fluid could be forced out of the damaged area.
• Try to stem the leak using tape or other waterproof material: No. This fluid would be under high pressure and will pass through a person's skin causing serious injury.
• Get some water, and wash the spilt hydraulic fluid away: No. the spill should be contained but attempting to wash it away will spread the oil. making the ground slippery over a wide area and increasing the chances of the oil making its way into the watercourse.
• Call a fitter and contain the escaping fluid by means of a spill kit or other facility: Yes, under no account should the driver put him/herself at risk by attempting a repair, instead they should contain the escaping fluid which would otherwise be an environmental hazard. The oil should be contained or minimised by deploying a spill kit or creating a physical barrier with earth etc.
